Confirm which O-1 sub-category applies: O-1A for sciences/education/business/athletics, or O-1B for arts/film/TV. Gather evidence demonstrating extraordinary ability/achievement.
For O-1B arts, obtain a written advisory opinion from an appropriate peer group, labor organization, or recognized expert. For O-1A and O-1B film/TV, this is optional but recommended.
The US employer or agent files Form I-129 (Petition for Nonimmigrant Worker) with the O supplement. Include all evidence, itinerary, and contract/agreement for the proposed event/employment.

If abroad: schedule DS-160 visa appointment at US Embassy/Consulate. If in the US on another valid status: file to change status without leaving, or depart and return on new O-1 visa.
O-1 is granted for up to 3 years initially. Extensions can be filed in 1-year increments without limit. File extension well before the current status expires.
